Adult Day Care

 

A supportive and therapeutic social program of supervision and care provided in an out-of-home setting to eligible adults during a part of a day, but less than twenty-four (24) hour care.  Planned activities are provided in a group setting, if appropriate, snacks and a noonday meal are provided.
 

Eligibility Criteria:

  • A person age 60 plus who is physically disabled or frail as a result of medical condition or age and who needs supervision or assistance during part of the day.
  • A person age 60 plus who is mentally confused and needs supervision to prevent injury and assure proper nutrition and medication use.
  • A person age 60 plus who, because of emotional or social needs, could benefit from the individualized attention and the social structure available through these services, which are not otherwise available.
  • A person or any age with diagnosis of probable Alzheimer’s Disease or related dementia disorder, as confirmed by a written statement from a physician after a diagnostic evaluation.

 

Cost of Services:

Cost for adult day care services are based on a sliding fee scale.  During the initial assessment, the case manager will review client income and medical expenses to determine and adjusted income to be used in determining cost for services.
